Please do not hesitate to get in touch with me at firstname.lastname@example.org if these times are strictly unsuitable. Your health is my priority and wherever possible I will endeavour to expand my hours to see you.
When booking, please include a message or comment with some information on your diet and nutrition goals to ensure I am appropriately prepared for our consultation.
What to bring to your consultation:
Please bring any recent blood tests and a list of the medications, supplements, and tinctures/herbs you’re currently taking.
Please contact Lifestrong for up-to-date clinic for fees, or email me if you’re interested in online. Fees may vary depending on the complexity of your condition and time spent in consultation with you.
If you have a chronic or terminal medical condition (a condition which has lasted or is likely to last 6 months or more) talk to your doctor about your eligibility for a Chronic Disease Management Plan and Enhanced Primary Care Plan, which entities you to a rebate of $53.80 for up to 5 sessions with a registered allied health provider, including dietitian.
Some gap fees may apply, please contact the clinic you’re interested in directly for more information. If you are experiencing financial hardship please advise your GP and the clinic and we will endeavor to accommodate your needs. I am also more than happy to have these conversations with you in my office.
If you are intending to obtain medicare funding this must be done before your appointment with your allied health provider, as plans cannot be backdated and failure to do so will lead to out-of-pocket expenses for you.
Please be aware that Medicare does not fund online services which means unfortunately this rebate is only available for face-to-face appointments.
Private Health Funds:
Many private health insurers provide rebates for nutrition and dietetics.
HICAPS terminal available on site to process the claim for you immediately, so you’ll just need to pay the gap fee on the day.
For my online service, some health insurers may allow you to claim online appointments while others will not, enquiries should be made with your health fund directly.
